Process to claim SINOT death benefits

• The employer must complete the Employer’s Report on the Application for Death Benefits
• The employer will give the claimant and dependent of the deceased worker:
  • List of documents required to claim death benefits
  • The Application for Death Benefits with the full Employer’s Report
• The claimant must call Triple-S Vida:
  • To request and schedule an appointment to deliver to our offices the documents required by the SINOT death claim. The appointment may be requested via email or by telephone:

•  We will not accept incomplete claims or those sent by mail. All claim documents must be delivered to one of our service offices by appointment.
